Anchor Health is seeking a compassionate Hospice RN Case Manager to initiate and coordinate individualized hospice care plans in patients' homes and communities. You will assess patients, develop and adjust plans, and work with the interdisciplinary team to meet physical, emotional, and spiritual needs.

The role requires an active CA RN license, CPR, and reliable transportation; experience in hospice and community health education is valued as you travel to patient locations across assigned

Qualifications

• RN license required.
• Hospice care experience preferred (1 year).

Responsabilités

• Assess and manage hospice patients and coordinate the plan of care.
• Coordinate patient care across the interdisciplinary team and with families.
• Communicate with physicians and provide education and support to patients and families.
• Ensure safe, effective nursing care and documentation of patient status and outcomes.
