Job summary

Anchor Health is seeking a Hospice Registered Nurse to conduct comprehensive patient and family assessments, develop and adjust care plans, and administer treatments in home hospice settings.

The role requires an active California RN license, CPR certification, and strong observation and communication skills, with preferred experience in home care or hospice nursing. A valid CA driver license and reliable vehicle are essential.

Qualifications

• Graduate of accredited school of nursing.
• Current RN license in California.
• Current CPR Certificate.
• Minimum experience: RN with 1 year in last 3 years or 2+ years in public, home care, or hospice nursing; BSN acceptable.
• Valid CA driver's license with insured automobile in good standing.
• Excellent observation and communication skills.

Responsibilities

• Perform initial, comprehensive patient and family assessments to determine hospice needs.
• Provide comprehensive physical assessment and medical history.
• Conduct home visits and evaluate patient status.
• Initiate and revise the plan of care as patient needs change.
• Develop care plans with nursing diagnoses and family involvement.
• Administer medications and treatments as prescribed.
• Participate in Interdisciplinary Group Meetings.
• Complete and submit accurate clinical notes on patient condition and care.
• Supervise ancillary staff and delegate tasks as required.
