Job summary

Anchor Health seeks a qualified hospice registered nurse to plan, organize and direct patient care in home settings. You will collaborate with the Interdisciplinary team and family to implement the patient care plan and ensure clear communication with physicians.

Requirements include CA RN licensure and CPR certification, with 1 year of recent professional nursing experience or a BSN with active RN license; two years of relevant public health or home care experience is preferred.

Qualifications

• Graduate of a National League of Nursing accredited school of nursing.
• Current licensure in State of CA and CPR certification.
• RN with within the last 3 years experience or a BSN with an active RN license.

Responsibilities

• Provides professional nursing care to hospice patients using the nursing process.
• Acts as Case Manager coordinating patient care for assigned caseload.
• Communicates with physicians regarding patient needs and orders.
• Coordinates care with community health professionals and family members.
