Job summary

Anchor Health in California is seeking a Hospice Registered Nurse to perform comprehensive patient and family assessments, develop and revise plans of care, and deliver in-home nursing services with a focus on comfort and quality of life.

You will administer medications, participate in interdisciplinary rounds, document care, and mentor ancillary staff, with driving to patient homes required. California RN license, CPR, and hospice/home health experience preferred.

Qualifications

• Graduate of an accredited nursing program.
• Current CA RN license.
• CPR certification is required.
• Two years of nursing experience in public, home care, or hospice is preferred.
• Valid CA driver's license and insured vehicle.
• Excellent observation and communication skills.

Responsibilities

• Complete comprehensive patient and family assessments to determine hospice needs.
• Conduct home visits and assess patient status with a focus on comfort.
• Develop and revise plans of care based on patient changes.
• Administer medications and treatments as prescribed by the physician.
• Participate in Interdisciplinary Group Meetings and document care.
• Supervise ancillary personnel when required and delegate tasks.
