Anchor Health in California is seeking a compassionate Hospice Team Nurse to plan, organize and direct home-based hospice care for each patient and family. You will assess patients, develop and revise care plans with the family, coordinate with physicians and the interdisciplinary team, and document care and outcomes.

CA RN license and CPR are required; hospice or public health experience preferred; a valid driver’s license is a must.

Qualifikationen

• Graduate of a NLN-accredited nursing program.
• Current CA RN license and CPR certification.
• Minimum one year experience as a professional nurse within the last three years, OR a BSN in nursing from a NLN-accredited program and a current RN license.
• Minimum of two years’ experience, at least one in public health, home care, or hospice nursing is preferred.
• Must be able to lift, position and transfer patients; strong observation, verbal and written communication skills.

Aufgaben

• Provide professional nursing care to patients using the nursing process.
• Assess and evaluate patient status.
• Initiate the plan of care and revise as patient needs change.
• Develop a care plan with goals based on nursing diagnoses and involve patient and family.
• Acts as Case Manager when assigned by Clinical Supervisor and coordinates patient care.
• Complete, maintain and submit accurate clinical notes; document pain/symptom management changes.
• Communicate with physicians and community health professionals to coordinate care.
