Qualifications

• Registered Nurse with valid CA state license.
• CPR certification current.
• Reliable transportation with a valid driver’s license.

Responsibilities

• Coordinate and oversee hospice care plan for each patient.
• Perform comprehensive and ongoing patient assessments.
• Administer medications and treatments per physician plan.
• Educate patient and family on care needs and self-care.
• Develop individualized goals with the interdisciplinary team.
• Communicate changes to physicians and update care plan.
• Ensure documentation and notes are accurate and timely.
