Gentiva Hospice is seeking a dedicated RN Case Manager to lead compassionate in-home hospice care across patients and families. You will assess, plan, and document care, coordinating with physicians, CNAs, social workers, and chaplains to ensure comfort and dignity.

You will educate families on medications, disease progression, and end-of-life steps while maintaining accurate records and adhering to regulatory standards, infection control, and quality initiatives.

Qualificações

• RN license in state of employment
• Hospice or palliative care preferred
• 1+ year RN experience in relevant settings
• CPR certification and driving requirement

Responsabilidades

• Perform comprehensive patient assessments and reassessments.
• Deliver skilled nursing interventions and symptom management.
• Develop and update individualized plans of care.
• Coordinate care with IDG and physicians.
• Educate patient and caregivers on disease, care, and meds.
• Document timely clinical records and care plans.
• Participate in on-call and patient support activities.
