Job summary

Gentiva is seeking a dedicated Hospice RN / RN Case Manager to guide patients and families through the hospice journey. You will deliver skilled nursing care in patients' homes or alternative settings, coordinate with an interdisciplinary team, and provide education and bereavement support as needed.

Qualifications

• Active RN license in the state of employment.
• Minimum 1 year RN experience in hospice, home health, or related fields preferred.
• CPR certification and current auto insurance/driver's license.

Responsibilities

• Be primary contact for patients and families through the hospice journey.
• Deliver and document skilled nursing care per Plan of Care at home or in alternative settings.
• Perform initial and ongoing assessments and adjust care as needed.
• Collaborate with IDG to refine personalized care plans.
• Provide education and support to patients, families, and caregivers.
• Ensure timely, accurate clinical documentation and infection control compliance.
• Support scheduling, quality improvement, and transitions between care levels.
• Communicate changes in condition to the care team and other providers.
