Job summary

Gentiva is seeking a compassionate Hospice RN / Case Manager to provide expert nursing care at patients' homes or equivalent settings. You will lead assessments, develop and update plans of care, coordinate with physicians and the IDG, and educate families on disease processes, medications, and end-of-life care.

You will support emotional and spiritual well-being, document thoroughly, and participate in on-call activities as needed, ensuring compassionate, compliant care across the hospice team.

Qualifications

• RN license in good standing in the state of employment.
• Minimum 1 year of RN experience in hospice, palliative care, home health, or related setting.
• CPR certification and driving eligibility with valid license.

Responsibilities

• Be the primary contact for patients and families throughout hospice care.
• Deliver skilled nursing care per Plan of Care in patients' homes or equivalent settings.
• Perform initial and ongoing patient assessments and adjust care as needed.
• Collaborate with IDG team to refine personalized care plans.
• Provide education, emotional, and bereavement support to families.
• Educate caregivers and ensure safe home care practices.
• Communicate patient status updates to care team for timely interventions.
• Maintain accurate clinical documentation and comply with standards.
• Support quality improvement and scheduling initiatives.
• Explain hospice services, medications, and end-of-life processes to families.
