Job summary

Gentiva Hospice is seeking a dedicated RN Case Manager to lead compassionate end-of-life care in patients’ homes and other settings. You will assess, plan, and coordinate personalized care with an interdisciplinary team, while educating patients, families, and caregivers and ensuring accurate documentation.

You will collaborate with physicians, LPNs, CNAs, social workers, chaplains, and others to implement plans of care and provide support across physical, emotional, and spiritual needs.

Qualifications

• Current license to practice nursing in the state of requested employment.
• CPR Certification.
• Current automobile insurance and valid driver’s license.
• Hospice or palliative care experience preferred
• A minimum of one (1) year experience as an RN, either in acute care hospital, oncology, long-term care, hospice or home health. School internships and clinical hours may be considered as experience in individual cases.

Responsibilities

• Be the primary point of contact for patients and families, guiding them through every step of their hospice journey.
• Deliver skilled nursing interventions and symptom management aligned with goals of care.
• Develop and update individualized plans of care including patient visit frequency to meet needs.
• Coordinate care across interdisciplinary teams including physicians and nurses.
• Provide patient and caregiver education regarding disease process, end-of-life care, medications, nutrition and dietary needs.
• Document timely clinical records on all patients including screening/assessment findings, physician orders, progress notes, and care plans.
• Maintain effective working relationships with IDG and involve team when care plans warrant team participation.
