Gentiva Hospice is seeking a dedicated RN Case Manager to lead compassionate end-of-life care in patients’ homes and other settings. You will assess needs, develop care plans, and coordinate with physicians, social workers, and the care team to ensure comfort and dignity.

You will educate patients and families, monitor symptoms, document care, and participate in on-call coverage as part of a team focused on quality, empathy, and compliance with regulatory standards.

Qualifications

• Valid RN license in the state of employment.
• Minimum 1 year of RN experience in acute care, oncology, long-term care, hospice or home health; hospice experience preferred.

Responsabilités

• Serve as primary contact for patients and families, guiding them through hospice care.
• Deliver and document skilled nursing care per Plan of Care in patients' homes.
• Perform initial and ongoing patient assessments and adjust care as needed.
• Collaborate with an interdisciplinary team to develop personalized care plans.
• Provide education, bereavement support, and caregiver training.
• Maintain accurate clinical documentation and comply with infection control standards.
• Participate in on-call and patient support activities.
