Job summary

Gentiva Hospice is seeking a compassionate Hospice RN / RN Case Manager to lead patient care in home environments, guiding families through the hospice journey with skilled nursing, education, and support. You will perform assessments, develop plans of care, and collaborate with an interdisciplinary team to ensure dignified, comfortable end-of-life care.

This role emphasizes patient- and family-centered care, adherence to regulatory standards, and proactive communication to address evolving

Qualifications

• Current license to practice nursing in the state of requested employment.
• CPR Certification.
• Current automobile insurance and valid driver’s license.
• Minimum of one year experience as an RN in hospice/home health or related field.
• Hospice or palliative care experience preferred.

Responsibilities

• Be the primary point of contact for patients and families through the hospice journey.
• Deliver and document skilled nursing care based on the Plan of Care in home settings.
• Perform initial and ongoing assessments and adjust care as needed.
• Collaborate with interdisciplinary team to refine care plans.
• Provide education, support, and bereavement guidance to patients and families.
• Teach caregivers practical nursing skills and safety practices.
• Communicate patient condition updates to the care team for timely interventions.
• Maintain accurate clinical documentation and comply with infection control standards.
• Support scheduling and quality improvement initiatives to ensure compassionate care.
• Navigate topics such as medication administration and hospice services with patients.
