Job summary

Gentiva is seeking a Hospice RN / RN Case Manager to lead patient-focused end-of-life care in home settings. You will assess, plan, and document nursing care, coordinating with physicians, LPNs, CNAs, social workers, and chaplains to ensure compassionate, personalized plans.

You will educate families, support emotional and spiritual needs, and ensure timely communication and documentation while adhering to regulatory standards and infection control. A rewarding role with meaningful impact.

Qualifications

• Active RN license in the state of employment.
• Minimum 1 year of RN experience in hospice, home health, or related setting.
• Hospice or palliative care experience preferred.
• CPR certification.
• Valid driver’s license and automobile insurance.

Responsibilities

• Be the primary point of contact for patients and families, guiding them through the hospice journey.
• Deliver and document skilled nursing care based on the Plan of Care—home or other care settings.
• Perform initial and ongoing assessments and adjust care as needed.
• Collaborate with an interdisciplinary team to refine personalized care plans.
• Provide emotional, physical, and spiritual support to patients and loved ones.
• Educate caregivers on safe, confident caregiving and practical skills.
• Communicate updates in patient condition to the care team for timely interventions.
• Maintain accurate clinical documentation and follow infection control standards.
• Support quality improvement and scheduling initiatives to ensure consistent, compassionate care.
• Explain topics such as medications, hospice philosophy, symptom management, and end-of-life processes.
