Stillwater Hospice is seeking a dedicated RN Case Manager in Fort Wayne/Allen County to provide comprehensive in-home hospice nursing care. This full-time role covers Fort Wayne and surrounding areas, with daytime hours, and includes weekend rotations.

Compensation is based on total years of experience in prior clinical roles. You will assess patients, develop and revise care plans, supervise care staff, and coordinate with the interdisciplinary team to ensure high-quality end-of-life care.

Formación

• Graduate of an accredited school of nursing.
• Must have a current RN license to practice in Indiana.
• CPR certification or obtain within orientation period.
• Prefer 1 year of hospital or home health care experience.
• Must have a valid driver’s license and access to a car with insurance.

Responsabilidades

• Manage and coordinate nursing care for hospice patients in home settings.
• Conduct initial assessment visits and identify health problems.
• Implement and revise the interdisciplinary plan of care; evaluate outcomes.
• Supervise LPNs, CNAs, and other care staff; delegate tasks.
• Document findings accurately and timely in patient records.
• Provide pain/symptom management, teaching, and advocacy.
• Participate in care conferences; coordinate with other providers.
• Be available for 24-hour call and holiday rotations; participate in on-call as assigned.

Conocimientos

CPR certification or withinOrientation

Strong interpersonal skills

Ability to drive to patient homes

Educación

Bachelor’s degree in nursing

Graduate of accredited nursing program

RN license in Indiana
