Stillwater Hospice is seeking a dedicated RN Case Manager to coordinate nursing care for terminally ill patients in home settings in the Warsaw area. The role involves assessing patient needs, implementing care plans, and evaluating outcomes within an interdisciplinary team.

Applicants should be an accredited nursing graduate with Indiana RN licensure; CPR certification required. Willingness to drive to patient homes and flexibility for weekend rotations are essential.

Qualifications

• Graduate of an accredited school of nursing.
• Bachelor’s degree in nursing preferred.
• Must have a current RN license to practice in the State of Indiana.
• Must hold current CPR certification or obtain within the orientation period.
• Must be able to provide those services requiring substantial specialized nursing skills.
• Prefer one (1) year of recent hospital or home health care experience.
• Must have a current motor vehicle operator’s license and have access to an automobile with current automobile insurance coverage.
• Must be able to drive independently to all patient/family clients’ homes.
• Must be able to safely perform the essential functions of the job without posing a direct threat to the health and/or safety of him/her, or to others in the workplace.
• Must be comfortable working with hospice patients with an emphasis on palliative care.

Responsabilités

• Manages and coordinates nursing care for assigned terminally ill patients by assessing needs, implementing nursing care plans, making revisions, and evaluating care outcomes.
• Performs initial assessment and uses OASIS assessments as appropriate.
• Delivers nursing care including pain management, teaching, case management and referrals; collaborates with the interdisciplinary team.
• Supervises LPNs, Home Health Aides, CNAs, and other aides; provides instruction on care procedures.
• Maintains clinical documentation accurately and timely; participates in on-call and holiday rotation.
• Adheres to agency policies and privacy practices; supports quality improvement initiatives.
