Heart to Heart Hospice in Fort Wayne, IN is seeking an RN Case Manager to deliver compassionate in-home and community-based care. You will perform comprehensive assessments, implement the care plan, and collaborate with physicians and a multidisciplinary team to meet patient needs at life-limiting stages.

You will participate in IDG meetings, coordinate care, and provide on-call coverage as needed. We offer competitive pay and full-time benefits to support professionals devoted to quality

Formación

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
• Current State Registered Nurse License.
• Current Driver's License with reliable transportation and auto liability insurance.
• Two years of RN experience preferred in healthcare/clinical settings.

Responsabilidades

• Conduct initial and ongoing assessments of patients' needs and ADLs.
• Implement and update the Plan of Care; revise as needed.
• Collaborate with physicians and healthcare professionals.
• Attend IDG meetings and coordinate care planning.
• Provide on-call service to patients and families.
• Ensure timely documentation and compliance with laws and policies.
• Performance of other duties as required.
