Qualifications

• Graduate of accredited school of nursing; RN license required in Indiana.
• Bachelor’s degree in Nursing preferred.
• Current CPR certification or able to obtain during orientation.
• Infusion skills are preferred.
• Strong interpersonal skills and ability to work with hospice patients and families.

Responsibilities

• Provide nursing care to terminally ill patients at the Hospice Home in Fort Wayne.
• Assess patient needs and coordinate interdisciplinary care plans.
• Document findings accurately and timely.
• Deliver direct nursing care including pain and symptom management.
• Participate in discharge planning and family education.
• Collaborate with physicians and other team members for patient-centered care.
