Job summary

Stillwater Hospice in Fort Wayne, IN is seeking an Inpatient RN for the hospice home facility. This full-time role offers 30 hours per week, Monday through Friday with weekend rotations, daytime hours, and an Excellent Work/Life Balance.

The RN will provide nursing care for terminally ill patients, coordinate care with the interdisciplinary team, document care, and support families. Benefits include health, dental, vision, retirement, PTO and tuition assistance.

Qualifications

• RN license to practice in Indiana
• One year of hospital or home health care experience preferred
• CPR certification required or obtained during orientation

Responsibilities

• Deliver nursing care to hospice patients, including pain and symptom management
• Coordinate care with physicians, LPNs, CNAs and other team members
• Document findings accurately and timely
• Orient patients and families to hospice services and plan of care
• Plan and implement discharge plans in collaboration with the care team
