Stillwater Hospice is seeking an RN Case Manager to coordinate nursing care for terminally ill patients in home settings across Fort Wayne and Allen County, IN. The role requires an Indiana RN license, CPR certification, and strong interpersonal skills.

The position involves managing the interdisciplinary plan of care, conducting initial assessments, and supervising nursing staff. Candidates should have a nursing degree (preferably BSN) and experience in hospital or home health care.

Competenze

• Graduate of an accredited school of nursing.
• Bachelor’s degree in nursing preferred.
• Must have a current RN license to practice in the State of Indiana.
• Must hold current CPR certification or obtain within the orientation period.
• Prefer one (1) year of recent hospital or home health care experience.
• Must have a current motor vehicle operator’s license and have access to an automobile with current automobile insurance coverage.
• Must be able to drive independently to all patient/family clients’ homes.

Mansioni

• Manages and coordinates nursing care for hospice patients in home settings.
• Implements and revises the interdisciplinary plan of care and evaluates outcomes.
• Conducts initial assessments and OASIS assessments as applicable.
• Supervises LPNs, Home Health Aides, CNAs, and other care staff.
• Participates in on-call and holiday rotations and collaborates with the clinical team to ensure quality care.
