Job summary

Stillwater Hospice is seeking an LPN Phone Nurse to support the clinical team in Fort Wayne. You will handle patient calls, assist with coordinating care, and provide nursing services across our service area.

The role requires an Indiana LPN or RN license, at least one year of hospital or home health care experience, and the ability to travel to patient homes. Typical schedule is 40 hours weekly, Monday through Friday with weekend rotations.

Qualifications

• Graduate of an accredited school of nursing; RN or LPN license required.
• Current Indiana LPN or RN license to practice.
• Minimum one year hospital or home health care experience.
• Ability to drive and reliable transportation.

Responsibilities

• Receives and responds to patient-related telephone calls.
• Recognizes needs and emergencies of hospice and palliative care patients.
• Coordinates care and communications with physicians and the interdisciplinary team.
• Delivers nursing care to patients in their homes and tracks patient condition.
