Job summary

Stillwater Hospice in Warsaw, Indiana seeks an RN Case Manager to coordinate nursing care for terminally ill patients in home settings by assessing needs, implementing and revising care plans, and evaluating outcomes.

Applicant must be a graduate of an accredited nursing program with Indiana RN license; CPR certification (or within orientation); Bachelor’s degree preferred; 40 hours/week, Mon-Fri with weekend rotation; competitive benefits and work-life balance.

Qualifications

• Must hold a current RN license to practice in Indiana.
• Bachelor’s degree in nursing preferred.
• CPR certification or within orientation period.
• Minimum 1 year of hospital or home health care experience preferred.

Responsibilities

• Coordinate and deliver nursing care for terminally ill patients at home.
• Assess patient needs, implement and revise care plans, and evaluate outcomes.
• Document findings clearly and timely in patient records.
• Lead or participate in interdisciplinary care conferences.
• Provide education and support to patients and families, including bereavement support when appropriate.
