Job summary

Affinity Hospice Management LLC in New Jersey seeks a Registered Nurse to plan, organize, and direct hospice care in patients' homes and communities. You will conduct initial and ongoing assessments, develop and revise care plans, administer prescribed treatments, and educate patients and families.

This role requires strong communication, the ability to work with an interdisciplinary team, strict adherence to nursing standards, a valid RN license, CPR certification, and a valid driver's license

Qualifications

• Graduate of an accredited school of nursing.
• Current RN license in good standing.
• 1 year of professional nursing experience within the last 3 years, or a baccalaureate degree in nursing with minimum two years’ experience (preferably in public health, home care, or hospice).
• Management experience not required; responsible for supervising hospice aides.
• Upon offer, satisfactory references and criminal background checks are required.
• Excellent observation, verbal and written communication skills; problem solving and basic math skills.
• Must be a licensed driver with insured vehicle in good standing.
• CPR certification required (hands-on verification required).

Responsibilities

• Completes initial and ongoing comprehensive patient and family assessments to determine hospice needs.
• Provides professional nursing care using the nursing process.
• Develops and revises the plan of care based on patient status and needs.
• Administers medications and treatments as prescribed.
• Counsels patient and family in meeting nursing and related needs.
• Provides health care instructions and supports activities of daily living.
• Acts as Case Manager when assigned and coordinates patient care.
• Maintains accurate clinical notes and communications with physicians and the care team.
• Participates in on-call duties and ensures resources and equipment are available.
• Contributes to quality assessment and performance improvement activities.
