Job summary

Affinity Hospice Management LLC is seeking a Registered Nurse to plan, organize, and direct hospice care, emphasizing community health education and in-home services. The RN will build from community resources to design and deliver tailored care for patients and families in their homes and communities.

In this role, you will perform comprehensive assessments, create and adjust the care plan, administer medications, provide patient and family education, and act as Case Manager when assigned.

Qualifications

• Graduate of an accredited school of nursing.
• RN license in good standing to practice.
• Minimum of one year of professional nursing experience in the last three years is preferred.
• Supervisory experience not required but ability to supervise hospice aides.
• Must pass background checks and references.

Responsibilities

• Complete initial and ongoing comprehensive patient and family assessments to determine hospice needs.
• Provide professional nursing care using the nursing process.
• Develop and revise the plan of care with patient and family input.
• Administer medications and treatments as prescribed.
• Counsel patients and families and provide health education.
• Assist with activities of daily living and promote self-sufficiency.
• Coordinate patient care as Case Manager when assigned.
• Maintain accurate clinical notes and communicate with physicians.
