Job summary

Affinity Hospice Holdings, LLC is seeking a registered nurse/case manager to plan, organize, direct, and provide hospice care in patients' homes and communities. The role emphasizes community health education and coordinating care across the interdisciplinary team.

The RN will build from community resources to meet patient and family needs, with PRN hours and home-based visits as required. Strong communication and nursing skills are essential to ensure quality end-of-life care.

Qualifications

• Graduate of an accredited nursing program.
• Current State license as a Registered Nurse.
• Current CPR certification.
• Minimum of two years of RN experience in a clinical care, home health or hospice setting preferred.
• Must be a licensed driver with insured vehicle.
• Excellent observation, verbal and written communication skills; nursing skills per competency checklist.
• Ability to lift, position and/or transfer patients.
• Ability to lift supplies and equipment.

Responsibilities

• Case manages and provides clinical direction to the interdisciplinary team, physician, and family.
• Implement/develop/document the patient plan of care
• Provide care utilizing infection control measures that protect both the staff and the patient (OSHA).
• Assure continuity of quality patient care delivered with appropriate documentation.
• Monitor assigned cases to ensure compliance with requirements of third-party payer.
• Demonstrate commitment, professional growth, and competency.
• Promote agency philosophy and administrative policies.
• Perform on-call responsibilities and provide on-call service to patients/families as assigned.
• Responsible for supervising hospice aides and licensed practical nurses.
• Perform other nursing duties as assigned by management to support department goals and initiatives.
