Job summary

Infinity Hospice Care LLC in Weatherford, TX is seeking an RN Case Manager to admit patients and provide nursing care at patients' homes. You will coordinate care with the Interdisciplinary Group and family, develop plans of care, and ensure documentation and regulatory compliance.

The ideal candidate has 2 years RN experience in clinical settings and at least 1 year in hospice, a current RN license, a valid driver’s license, and strong communication skills to support patients and their families.

Qualifications

• 2 years RN experience in a clinical care setting.
• 1 year full-time hospice experience preferred.
• Current State RN license; valid driver’s license.

Responsibilities

• Assess hospice patients using Nursing Assessment and LCD criteria to identify needs.
• Develop and document the plan of care aligned with assessment.
• Provide nursing care with infection control measures.
• Collaborate with Interdisciplinary Group/Team to meet patient/family needs.
• Communicate with attending and hospice physicians involved in care.
• Submit required paperwork in a timely manner.
• Participate in hospice in-service programs and QAPI activities.
• Promote Hospice policies and philosophy; meet productivity standards.
• Provide on-call services to patients and families as assigned.
• Maintain effective communication with patients, families, and health care professionals.
