Job summary

Infinity Hospice Care LLC is seeking a Registered Nurse to perform admissions and administer nursing care for patients in their homes. You will coordinate with the interdisciplinary team, patient/family, and referring agencies to ensure comprehensive care.

Responsibilities include care planning, on-call duties, timely paperwork, and maintaining infection control standards. A valid RN license and driver’s license, plus hospice experience, are required.

Qualifications

• Two years RN experience in a clinical care setting.
• One year full-time hospice experience.
• Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing.
• Current State RN license and valid driver's license.
• Knowledge of regulations and good interpersonal skills.

Responsibilities

• Provide admissions and nursing care for patients at home.
• Develop and document the plan of care.
• Coordinate with Interdisciplinary Group/Team and referrals.
• Communicate with attending physicians and hospice physicians.
• Submit required paperwork timely.
• Participate in hospice-sponsored in-service programs and QAPI.
• Maintain infection control standards and patient safety.
• Provide on-call nursing services as assigned.
• Ensure effective communication with patients and families.
• Travel to patient homes as needed.
