Job summary

Lifecare Home Health is seeking a Registered Nurse Case Manager to perform admissions and administer nursing care for patients in their homes. You will coordinate care with the Interdisciplinary Team, patient and family, and referring agencies to ensure comprehensive, timely services.

Requirements include an active RN license, 2 years of clinical RN experience, and hospice exposure. The role involves driving to patient homes and participating in on-call rotations as needed.

Qualifications

• RN license in good standing (Texas preferred).
• Two years as an RN in a clinical setting with hospice experience.
• Education: graduate of an accredited nursing program (Diploma/ADN/BSN).

Responsibilities

• Perform admissions and coordinate home-based nursing care.
• Assess hospice patients and develop the care plan.
• Coordinate with Interdisciplinary Group/Team and physicians.
• Ensure timely submission of required paperwork.
• Participate in Hospice QAPI program and on-call duties.
