Job summary

Infinity Hospice Care LLC seeks an experienced RN Case Manager to perform admissions and provide nursing care for patients in their homes. You will coordinate care with the Interdisciplinary Group/Team, patient and family, and referring agencies to ensure coordinated, compassionate care.

Responsibilities include assessing patients, documenting care plans, maintaining infection control, communicating with physicians, submitting paperwork on time, and participating in QAPI programs while meeting

Qualifications

• Two years RN experience in a clinical setting.
• One year full-time hospice experience.
• Current State RN license and a valid driver’s license.

Responsibilities

• Assess hospice patients using Nursing Assessment and LCD criteria.
• Develop and document the care plan in correlation with the assessment.
• Provide nursing care with infection control measures (OSHA).
• Coordinate with Interdisciplinary Group/Team to meet patient/family needs.
• Communicate with attending and hospice physicians and other clinicians.
• Submit required paperwork in a timely manner.
• Participate in hospice in-service programs and QAPI activities.
• Meet productivity standards and perform on-call duties.
• Maintain effective communication with patients, families, and staff.
• Travel to patient homes as needed; ensure reliable transportation.
