Job summary

Infinity Hospice Care LLC seeks an RN Case Manager to conduct admissions and provide nursing care for patients in their homes. You will coordinate with the interdisciplinary team, patients, families, and referrers to ensure comprehensive care and timely documentation.

Responsibilities include assessing needs, developing care plans, and meeting regulatory requirements while participating in QAPI and in-service programs. Travel to patient homes and flexibility in schedule are required.

Qualifications

• One year as an RN in a hospice setting.
• Current RN licensure and driver’s license.
• Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing.
• Knowledge of state and federal regulations and infection control.

Responsibilities

• Assess hospice patients and identify physical, psycho-social, and environmental needs.
• Develop and document the plan of care in correlation with the assessment.
• Provide nursing care utilizing infection control measures (OSHA).
• Coordinate with Interdisciplinary Group/Team to meet patient/family needs.
• Communicate with attending physicians and hospice physicians involved in patient care.
• Submit all required paperwork in a timely manner.
• Participate in hospice in-services and QAPI program; maintain or improve competency.
• Promote hospice philosophy and policies.
• Meet productivity standards.
