Infinity Hospice Care LLC is seeking an RN Case Manager to perform admissions and provide nursing care for patients at home. You will coordinate with the interdisciplinary team, patients, families, and referring agencies to ensure a comprehensive care plan.

The role requires 2 years RN experience in clinical care and at least 1 year in hospice, valid Georgia RN license, and a driving license. Strong communication and adaptability are essential.

Competenze

• Two years RN experience in a clinical setting and one year in hospice.
• Graduate of an accredited nursing program and current Georgia RN license.
• Valid driver’s license and reliable transportation.
• Knowledge of state and federal regulations and OSHA infection control.

Mansioni

• Perform admissions and nursing care for patients at home.
• Coordinate care with the interdisciplinary team and family.
• Develop and document the plan of care based on assessment.
• Communicate with attending and hospice physicians involved in care.
