Zusammenfassung

Infinity Hospice Care LLC is seeking a skilled RN Case Manager to perform admissions and coordinate nursing care for patients at home. You will work with the Interdisciplinary Group/Team, patients and families to assess needs, develop plans of care, and ensure timely documentation.

The role requires current Texas RN license, hospice experience, ability to travel in the Dallas area, and participation in QAPI and in-service programs. A WellSky Consolo background is preferred.

Qualifikationen

• One year RN experience in hospice setting.
• WellSky Consolo experience preferred.
• Education: Nursing diploma/degree from accredited program.
• Licensure: Current State RN license; valid driver’s license.
• Knowledge of state and federal regulations; strong interpersonal skills.

Aufgaben

• Assess hospice patients using Nursing Assessment and LCD criteria to identify physical, psycho-social, and environmental needs.
• Develop and document the plan of care in correlation with the assessment.
• Provide nursing care with infection control measures.
• Collaborate with Interdisciplinary Group/Team to meet patient/family needs.
• Communicate with attending physicians, hospice physicians and other involved physicians.
• Submit required paperwork promptly.
• Participate in hospice in-service programs and QAPI activities.
• Promote hospice philosophy and policies.
• Meet productivity standards.
• Perform on-call responsibilities for patients and families.
• Provide effective communication to patients, families, team, and other health professionals.
• Maintain reliable transportation and valid auto liability insurance.
