Job summary

Infinity Hospice Care LLC is seeking an RN Case Manager to perform admissions and provide nursing care in patients' homes across Austin and surrounding areas.

You will collaborate with the Interdisciplinary Team, document plans of care, ensure regulatory compliance, and participate in QAPI programs. The role requires at least 2 years of RN experience and 1 year hospice, plus a valid Texas RN license and reliable transportation.

Qualifications

• Two years experience as a Registered Nurse in a clinical care setting and one year full time hospice.

Responsibilities

• Assess hospice patients using Nursing Assessments and LCD criteria to identify needs.
• Develop and document the plan of care.
• Provide nursing care with infection control measures (OSHA).
• Coordinate with Interdisciplinary Group/Team to meet patient/family needs.
• Communicate with attending physicians and hospice physicians involved in care.
• Submit all required paperwork in a timely manner.
• Participate in hospice in-service programs and QAPI.
