Infinity Hospice Care LLC seeks an RN Case Manager to perform admissions and provide nursing care for patients in their homes in Granbury, Texas. You will coordinate with the Interdisciplinary Group/Team and families to develop and document care plans while ensuring regulatory compliance.

The role requires strong nursing skills, excellent communication with physicians, and timely submission of paperwork. You will participate in in-service programs and uphold hospice quality initiatives.

Formación

• Two years of RN experience in a clinical care setting.
• At least one year of full-time hospice experience.
• Current State RN license and valid driver9s license.

Responsabilidades

• Admit patients and provide nursing care in patients' homes.
• Coordinate care with Interdisciplinary Group/Team and families.
• Communicate with physicians involved in patient care.
• Submit required paperwork in a timely manner.
• Participate in hospice-sponsored in-service programs and QAPI.
