Job summary
Tufts Medicine Care at Home is seeking an experienced Hospice RN Case Manager to serve as the case manager for the interdisciplinary team, delivering quality hospice nursing care per patient plans in Malden, MA and surrounding areas.
The role requires an active RN license, CPR (or BLS within 30 days depending on hire date), and 1 year of hospice/home health experience, with opportunities for professional growth and ongoing education.
Qualifications
- Active RN license in the state where services are provided.
- Current CPR Certification.
- One year of hospice and home health experience.
- If hired before 9/28/2026, CPR required on hire; if after, BLS within 30 days.
Responsibilities
- Provide hospice nursing care using current principles of palliative care and symptom management.
- Coordinate care in a cost-efficient manner.
- Collaborate effectively with other members of the IDT.
- Utilize nursing process and standards of practice in providing hospice care.
- Maintain competency in nursing skills and patient teaching.
- Communicate with physician and Tufts Medicine Care at Home staff regarding changes in patient condition.
- Understand family dynamics and support diverse family systems.
- Perform nursing visits and on-call/triage as needed.
