Responsabilités

• Coordinate hospice care with the patient, family and IDG.
• Maintain and update the patient care plan at least every 14 days.
• Communicate with physicians and team for end-of-life care.
• Ensure hospice eligibility and accurate EMR documentation.
• Provide bereavement support and family education as needed.
