Job summary
Tufts Medicine Care at Home, Inc. is seeking a Home Health Registered Nurse in Malden, MA to provide comprehensive in-home nursing care. You will perform assessments, interventions, and patient advocacy, coordinating with physicians and care teams to deliver safe, effective care.
The role requires an active RN license, a valid driver's license, and at least 1 year of acute care experience. BLS/CPR is required by hire date, with strong emphasis on preventing hospitalizations and supporting
Qualifications
- Active RN license in the state of practice; license must be current.
- Valid state-issued Driver's License.
- Minimum 1 year of acute medical/surgical nursing experience.
- BLS/CPR certification required by hire date.
Responsibilities
- Assess patients using the nursing process and document findings.
- Develop and implement care plans with patient/family involvement.
- Coordinate with care team and community resources for discharge planning.
- Reconcile medications and manage orders per physician’s instructions.
- Provide safe home care while maintaining own safety in home and community.
