Tufts Medicine Care at Home is seeking a Home Health Registered Nurse to manage and deliver comprehensive home health services in patients’ homes. The role includes assessments, interventions, and caregiver support to reduce hospitalization and ensure safe, effective care.

Strong communication with patients, families, and the care team is essential. Requirements include an active RN license, a valid driver’s license, and 1 year of acute nursing experience with BLS (CPR per hire date).

Formación

• Active RN license in the state of practice.
• Valid state Driver's License in good standing.
• 1 year of acute medical/surgical nursing experience preferred.
• Basic Life Support Certification (and CPR as required by hire date).

Responsabilidades

• Assess patients’ physical and psychosocial needs following Standards of Nursing Practice.
• Coordinate care and discharge planning with the health care team.
• Document patient data and care plans promptly in the electronic record.
• Provide nursing care in the patient’s home and support families/caregivers.
