Tufts Medicine Care at Home is seeking a Home Health Registered Nurse to manage comprehensive home health services in Malden, MA. You will assess patients, implement plans of care, and provide essential nursing care within the home setting, coordinating with physicians, families, and care teams.

The role requires an active RN license in MA, BLS, and at least 1 year of acute nursing experience. BSN is preferred, with a strong emphasis on safety, documentation, and patient advocacy.

Qualifications

• Active RN license in MA; out-of-state licensure allowed if obtained within 60 days.
• Driver’s license required.
• 1 year of acute medical/surgical nursing experience.
• BLS required; CPR if hired after 9/28/2026 with 30-day deadline.

Responsibilities

• Assess patient needs and develop care plans with the interdisciplinary team.
• Perform nursing duties per physician orders and standards of practice.
• Maintain accurate, timely documentation in patient records.
• Coordinate care and discharge planning with providers and family.
• Provide patient and family education and advocacy.
• Deliver care while ensuring safety in the home environment.
