Job summary

Tufts Medicine Care at Home is seeking a Home Health Registered Nurse to manage and deliver comprehensive home health services, including assessments, interventions, and supportive care to clients in their residence. The role emphasizes patient advocacy, safety in the home, and collaboration with a multidisciplinary team.

Required are an active RN license in-state, a valid driver’s license, and at least one year of acute medical/surgical nursing experience, with BLS and CPR as applicable.

Qualifications

• Active RN license for the state of service; licensure in other states must be obtained if required.
• Valid state issued Driver’s License.
• One year of acute medical/surgical nursing experience.
• Basic Life Support Certification.
• CPR/BL depending on hire date requirements.

Responsibilities

• Assess patients’ physical and psychosocial needs following standards and policies.
• Coordinate with patients/families and healthcare providers for care planning.
• Document patient data accurately in the electronic health record.
• Complete physicians’ orders and reconcile medications with patients and physicians.
• Provide nursing coverage evenings, nights, weekends and holidays as required.
• Ensure safety and effective discharge planning within team care delivery.
