Strong assessment and communication skills are essential to support patients and families. The position requires an active RN license and hospice/home health experience; CPR is required, with BLS within 30 days if hired before 9/28/2026.

Qualifications

• Active RN license in the state of practice.
• CPR certification current.
• 1 year hospice and home health experience preferred.
• CPR/BSL requirements per hire timeline.

Responsibilities

• Provide hospice nursing care aligned with palliative care principles.
• Coordinate care with the IDT to achieve patient plan of care.
• Update Care Plan and medication profiles regularly.
• Document nursing assessments, interventions and outcomes per standards.
