Job summary

Tufts Medicine Care at Home is seeking a Home Health Registered Nurse in Malden, MA. You will manage and deliver comprehensive in-home health services, including assessments, interventions, and patient advocacy, with emphasis on avoiding hospitalization.

Requirements include an active RN license, a valid driver's license, 1 year of acute nursing, and BLS; CPR within 30 days if hired after 9/28/2026. The role offers full-time hours and weekend/holiday coverage as needed.

Qualifications

• Active RN license in the practicing state; licensure in other states as required.
• Valid state driver's license.
• One year of acute medical/surgical nursing experience.
• Basic Life Support (BLS) certification; CPR required upon hire if before 9/28/2026 or within 30 days after.

Responsibilities

• Assess patients and families for physical and psychosocial needs using established standards.
• Develop and adjust care plans in collaboration with the health care team.
• Provide in-home nursing care, medication reconciliation and safety planning.
• Triage patient visits by priority and coordinate with physicians and caregivers.
• Complete OASIS and related orders; document care in the patient record.
• Support patient/caregiver education and advocacy; ensure safe home environment.
