Job summary

Tufts Medicine is seeking an experienced Home Health Registered Nurse to manage and deliver comprehensive home health services in clients’ residences in the Malden area. The role emphasizes assessments, interventions, and supportive care, with strong emphasis on avoiding hospitalization and collaborating with families and a broad health-care team.

The ideal candidate holds an active MA RN license, 1+ year acute medical/surgical experience, and BLS; BSN preferred.

Qualifications

• Active RN license in the state of service (Massachusetts); licensure must be maintained.
• Valid state Driver's License.
• Minimum 1 year of acute medical/surgical nursing experience.
• Basic Life Support (BLS) certification.
• If hired before 9/28/2026, CPR certification required at hire; otherwise within 30 days.

Responsibilities

• Assess patients’ physical and psychosocial needs per Standards of Nursing Practice and agency policies.
• Assess patient/family learning needs and tailor information delivery.
• Respect patients’ and caregivers’ cultural beliefs related to death and dying.
• Develop realistic goals and plans of care in collaboration with patient and family.
• Identify abuse/neglect and follow reporting mechanisms.
• Adapt to new situations without impacting performance; use security when needed.
• Collaborate with patient/family and other providers for care planning and discharge.
• Continuously assess/evaluate progress and justify continuation or termination of services.
• Document data, activities and communications accurately in the medical record.
• Complete physician orders, levels of care and OASIS; reconcile medications.
• Follow safety guidelines and proper body mechanics; ergonomic practices.
• Respond to emergencies using established procedures and stabilizing techniques.
• Provide compassionate support for patient/family emotional stress.
• Manage caseload within team care delivery; plan daily work based on priorities.
• Provide nursing coverage on evenings, nights, weekends and holidays.
• Demonstrate thorough assessment and care planning; current pharmacology knowledge.
• Coordinate care and discharge planning with team members.
• Maintain knowledge of payers and regulatory agency policies for documentation.
• Communicate with all disciplines regularly and during crises.
