Job summary

Affinity Hospice Management LLC seeks an experienced Registered Nurse to plan, organize, and direct hospice care in patients' homes and communities. The RN will draw on community resources to meet the needs of individuals and families and provide education on health and end-of-life care.

The ideal candidate has NLN-accredited nursing education, state RN licensure, and 1–2 years of professional nursing experience, with hospice/home health preferred.

Qualifications

• Graduate of NLN-accredited nursing program.
• Current licensure in State.
• 1 year as a professional nurse in last 3 years, or BSN with RN license.
• 2+ years’ experience, at least 1 in public health/home care/hospice preferred.

Responsibilities

• Perform initial and ongoing comprehensive assessments of patient and family.
• Provide professional nursing care using the nursing process.
• Develop and revise care plans with patient/family input.
• Administer medications and treatments as prescribed.
• Educate patient and family on self-care and home care needs.
• Document notes and communicate with physicians.
• Participate in on-call duties and coordinate equipment/services.
• Supervise hospice aides and ensure continuity of care.
