Job summary

Affinity Hospice Management LLC is seeking a Registered Nurse to plan, organize and direct hospice care, with a focus on community health education and home-based services. The RN will build from community resources to meet patient and family needs in home settings.

Responsibilities include comprehensive assessments, care planning, medication administration, patient/family counseling, and coordination with the interdisciplinary team.

Qualifications

• Graduate of a National League of Nursing accredited school of nursing.
• Current licensure in the state.
• RN with minimum 1 year in the last 3 years; or BSN degree with current RN license; 2 years total with at least 1 in public health/home care/hospice preferred.
• Management experience not required; may supervise hospice aides.

Responsibilities

• Complete initial, ongoing comprehensive assessment of patient and family.
• Provide professional nursing care using the nursing process.
• Develop and revise the plan of care with the interdisciplinary team.
• Administer medications and treatments as prescribed by the physician.
• Counsel patient and family and provide health care instructions.
• Act as Case Manager for assigned caseload.
• Participate in on-call duties and ensure equipment/services are available.
• Engage in quality assessment and performance improvement activities.
