Affinity Hospice Management LLC is seeking a Registered Nurse to plan, organize, and direct hospice care for patients in home and community settings. The RN will conduct comprehensive assessments, develop and revise care plans, and administer treatments under physician orders.

You will collaborate with an interdisciplinary team, educate patients and families, and participate in ongoing quality improvement activities while maintaining documentation and compliance with regulatory requirements.

Competenze

• Graduate of an accredited school of nursing.
• Registered Nurse with license in good standing.
• 1 year of professional nursing experience in last 3 years or 2 years with a BSN; preferred in public health/home/hospice.
• Management experience not required; may supervise aides.
• Background checks required after offer.
• Excellent observation, communication, problem solving and math skills.
• Mobility: walking/standing, lifting, reaching; visual/hearing ability.
• Licensed driver with insured vehicle; CPR certification.

• Complete initial and ongoing patient/family assessments to determine hospice needs.
• Provide professional nursing care using the nursing process.
• Assess and evaluate patient status and revise the plan of care as needed.
• Initiate and revise the plan of care based on patient needs.
• Develop a care plan based on nursing diagnosis and family involvement.
• Administer medications and treatments as prescribed.
• Counsel patient and family and provide health instruction.
• Assist with activities of daily living and comfort care as needed.
• Coordinate patient care as Case Manager when assigned; maintain notes and records.
• Communicate with physician and other health professionals; ensure orders are followed.
• Participate in on-call duties and arrange equipment/services as needed.
• Supervise hospice aides when required; contribute to quality improvement activities.
