Job summary

Affinity Health Management is seeking a registered nurse to plan, organize and direct hospice care, with experience in community health education. The nurse will build from community resources to plan and direct services for individuals and families in their homes and communities.

The role emphasizes patient assessment, care planning, coordination with physicians, and education for patients and families, including support with daily living and comfort measures in home hospice settings.

Qualifications

• Graduate of an accredited school of nursing.
• Licensed RN in good standing to practice.
• Minimum one year of professional nursing experience within the last three years; OR a baccalaureate degree in nursing with at least two years’ experience in public health, home care, or hospice preferred.
• Management experience not required; Responsible for supervising hospice aides.
• Criminal background checks and references may be requested after offer.
• Excellent observation, verbal and written communication skills, problem solving and basic math skills.

Responsibilities

• Complete initial, comprehensive and ongoing assessments to determine hospice needs.
• Provide professional nursing care using all elements of the nursing process.
• Develop and revise care plans based on patient status and nursing diagnosis.
• Initiate plan of care and make revisions as patient needs change.
• Administer medications and treatments as prescribed by the physician.
• Counsel patient and family in nursing needs and self-care techniques.
• Coordinate care with the interdisciplinary team and act as Case Manager when assigned.
• Participate in on-call duties and ensure equipment and services are available.
