Job summary

Gentiva is seeking a dedicated Hospice RN / RN Case Manager to provide personalized end‑of‑life care in patients' homes, assisted living, or other care settings in Memphis. You will assess, plan, and implement nursing care with an interdisciplinary team to support patients and families.

The role requires a valid RN license, CPR, and at least 1 year of hospice or related experience. It is a full‑time weekday position with a limited after hours on‑call rotation and generous benefits.

Qualifications

• Must hold an active RN license.
• CPR certification required and current.
• Minimum of 1 year RN experience in hospice, palliative care, home health, or acute care hospital.

Responsibilities

• Perform comprehensive patient assessments and ongoing reassessments.
• Deliver skilled nursing interventions and symptom management.
• Develop and update individualized plans of care and visit schedules.
• Coordinate care with physicians, LPNs, CNAs, social workers, and chaplains.
• Educate patients and caregivers on disease processes, medications, and end‑of‑life care.
