Qualifications

• Active RN license in the state of employment.
• Hospice experience preferred.
• Strong communication and assessment skills.

Responsibilities

• Lead patient and family through hospice journey and care planning.
• Deliver skilled nursing care in home or facility settings.
• Perform initial and ongoing patient assessments and adjust care as needed.
• Collaborate with physicians, LPNs, CNAs, social workers and chaplains.
• Provide education and bereavement support to families.
