Job summary

Gentiva is seeking a Hospice RN / Case Manager to deliver personalized end-of-life care in patients' homes and other settings. You will perform initial and ongoing assessments, develop and update plans of care, and work closely with physicians, social workers, and chaplains to ensure compassionate, high-quality care.

Responsibilities include educating families, managing symptoms, and coordinating with the IDG to ensure smooth transitions across levels of care.

Qualifications

• Current RN license in the state of employment.
• Hospice or palliative care experience preferred.
• Experience delivering home-based nursing care is expected.

Responsibilities

• Perform comprehensive patient assessments and reassessments in home settings.
• Deliver skilled nursing interventions and manage symptoms per plan of care.
• Coordinate care with interdisciplinary team to adjust plans as patient needs change.
• Educate patients and families on disease, medications, and care transitions.
• Document care activities and maintain accurate, timely records.
