Job summary

Gentiva Hospice is seeking an RN to provide skilled nursing care and case management in patients' homes or care settings in Chattanooga, TN. You will assess patients, implement plans of care, and collaborate with physicians, social workers, and chaplains to ensure compassionate, high‑quality end‑of‑life care.

Requirements include an active RN license and at least 1 year of RN experience in hospice, home health, or acute care, with CPR certification and a valid driver's license.

Qualifications

• Current license to practice nursing in the state of employment.
• Hospice or palliative care experience preferred.
• Minimum of one year RN experience in acute care, hospice, home health, or related settings.

Responsibilities

• Perform comprehensive patient assessments and reassessments in home or facility visits.
• Deliver skilled nursing interventions and symptom management per care plans.
• Develop and update individualized plans of care and visit frequency.
• Coordinate with interdisciplinary team to refine patient care plans.
• Educate patients and caregivers on disease, care techniques, and end‑of‑life issues.
